HomeMy WebLinkAboutOrdinance No. 245 ORDINANCE NO. ~ AN ORDINANCE OF THE CITY OF THE COLONY, TEXAS, AUTHORIZING THE CITY MANAGER TO ENTER INTO A CONTRACT WITH THE FIRM OF U~i'OI~ ZI'F~ --_~_yT~.~l~F$rl<% ~.~ IN THE AMOUNT OF"~Y~o~O~o ~'~ FOR CITY HEALTH INSURANCE; ATTACHING THE APPROVED FORM OF THE CONTRACT AS EXHIBIT 'IA"; AND PROVIDING AN EFFECTIVE DATE. BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF THE COLONY, TEXAS: SECTION 1. That the City Manager of the City of The Colony, Texas, is hereby authorized to execute on behalf of the City a contract with the firm of ~/~ Zr.~ ~N~. ~F~_~/ in the amount of~, ~_~, ~:~:~ for City health insurance. The approved form of such contract is attached hereto as Exhibit "A", and made a part hereof for such purposes. SECTION 2. This ordinance shall take effect immediately from and after its passage by the City Council of the City of The Colony, Texas. DILLARD III, CITY ATTORNEY AMENDMENT TO THE GROUP INSURANCE POLICY NO. 190 ISSUED TO CITY OF THE COLONY The above named policy is hereby amended to include the following: TR~-NSFERRED BENEFITS PROVISION If an individual incurs expenses which would otherwise be excluded by Section V, Employee and Dependent Benefits, Pre-existing condi- tions, V-4 of this policy, and if a. the individual is insured on the effective date of this policy, and b. the individual was insured on the immediately preceding day under any policy or plan which was replaced by this policy, and c. such expenses would have been payable under the policy or plan which was replaced by this policy, Union Life will pay the lesser of the total amount payable for the excluded expenses under d. the policy or plan replaced, or e. this policy without Section v, Employee and Dependent Benefits, Pre-Existing conditions, V-4 in effect. This amendment shall be effective 12:01 A.M. Standard Time, March 1, 1983 at The Colony, Texas. CI UNION LIFE INSURANCE CoMpANy MEN Assistant Vice President GROUP INSURbJ~CE POLICY NO. 190 ISS~ TO CITY OF THE COLONY The Group Policy as herein issued will provide the following additional benefits: Upon the first day of the month following the death of an insured emoloyee and at the option of the deceased employee's spouse, Medical Expense Benefits for the spouse ~nd any eligible children may be continued, orovided such spouse and dependent children were insured prior to the death of the employee, up to the time the first of the following occurs: the end of a period of one year from the death of the employee, remarriage of the s-o2viving spouse, eligibility of the dependent for a~y other group health insurance coverage, the spouse's attainment of age 65, or dependent child's attainment of any limiting age. The maxi_mum age lfm~t of an employee's unmarried child(ren) has been extended to the date of attainment of age 26 providing the child(ren) is unmarried, maintains legal residence with the employee and is registered as a full-time student in an accredited college or university. SECTION V - EMPLOYEE AND DEPENDENT BENEFITS, ~i~JOR MEDICAL BENEFITS, Benefit Percentage, the following statement is hereb~ deleted: AFter ~ 2,500 ~ of covered charges p~yabie at 80%~ have been incurred within a calendar year, M~Jor Medical pays 100% of covered charges (other than those for (i) mental and nervous conditions and (ii) dental conditions) subse0uently_ incurred within the calendar year which are not required to satisfy a deductible. and the following shall apply: Stop Loss Benefit After $ 2,500 ~ of covered charges payable at 80% have been incurred within a calendar year, Ma~or Medical pays 100% of covered charges (other than those for (i) mental and nervous conditions and (ii) dental conditions) subsequently incurred within the calendar year which are not required to satisfy a deductible.~.: When three members of a family unit consisting of the employee and his dependents insured under this policy have been reimbursed at the 100% Stop Loss Benefit level in any one calendar year, all other f~.m~ly members shall be entitled to 100% reimbursement of covered medical expenses incurred during that same calendar year. IN ACCORDANCE WITH AND SUBJECT TO THE TERMS AND CONDITIONS OF THIS POLICY: The Union Life Insurance Company (herein called Union '' ox LIf~ in consideration of the application of the Policyholder and of the payment of premium due on the Date of Issue and Renewal Premiums as due agrees to provide group insurance benefits with respect to each individual insured on and after the Date of Issue. This policy will terminate upon failure to pay any premium before expiration of the grace period allowed for payment or following written notice by Union Life or the Policyholder.
